The forth sex scene is a lesbian scene, it is depicted as a artistic nude picture of a women being mirrored and then pushed against itself, Cleopatra moans in the background of this one.

A man molds Cleopatra's body into the most beautiful Egyptian women imaginable. He makes her more shapely and we see he makes her breasts more appealing.
