The Crew (2008)
The Crew is a 2008 American comedy film directed by Michael Dinner and starring Richard Dreyfuss, Burt Reynolds, and Seymour Cassel. The film follows four aging mobsters who reunite to pull off one last job. While the movie is primarily a comedy, it also contains some surprisingly steamy sexual moments.

There is a particularly disturbing and perverse scene where two heroin addicts, one who appears to have downs syndrome are sexually and brutally abused by two of the main characters, it's very graphic and harrowing to watch
Within just the first 15 minutes of the film you see a man and woman having sex (only breasts visible) but is ended when the woman realizes the man is not enthusiastic and also within just the first 15 minutes you see a man masturbating to a pornagraphic film (however the film is seen but very blurry)
